How can medical doctors begin referring to Lift for Life?

• Identify where Lift for Life programs are available. All licensed providers are listed on this website. It is a requirement of this license that providers only use trainers who have a current Lift for Life accreditation, providing you with assurance that your clients will get a quality program and the very best of care.
• Make contact with your local providers and discuss your interest in the program and the potential of referral of your practice’s clients. If there are currently no providers available in your local area, consider contacting your local health and fitness centre and discussing the introduction of the program. If the Centre is aware that the local doctor is interested in referring patients to them, they may be motivated to license the program.
• If there is a provider in your local area, download an Approval to Participate Form. Keep copies of this form on hand to enable ease of access and referral of patients looking to, or who need to improve their glycaemic control. Clients must take a fully completed Approval to Participate Form to their first session.

• Make contact with your local providers and discuss your interest in the program and the potential of referral of your clients. If there are currently no providers available in your local area, consider contacting your local health and fitness centre and discussing the introduction of the program. If the Centre is aware that a local Diabetes Educator is interested in referring patients to them, they may be motivated to license the program.
• If there is a provider in your local area, download the What is Lift for Life flyer and an Approval to Participate Form. Keep copies of these forms on hand and instruct those clients who you deem it appropriate to visit their GP to obtain an approval to participate. Clients must take a fully completed Approval to Participate Form to their first session.

Using Medical Director to make referrals

If you use Medical Director you can add a link to the Lift for Life referral form.

Follow these instructions carefully. Do not open the attached file in "Word". If it is opened in Word, it will lose its connection to the Medical Director database. Installation instructions

• Highlight the link, right click and choose "save target as...". Save the attached file to an easy-to-remember location on your computer, noting the filename (but DO NOT open the template file)
• Open Medical Director
• Open Letter Writer
• Go to the File menu and choose Modify Template
• Select Blank Template
• Go to the File menu and choose Import
• Locate the saved template file and double-click it
• Go to the File menu in Letter Writer and choose Save As Template
• Name the template and click the Save button.
• Open a test patient file and test the template
